Yes. The horizontal rungs are placed randomly, and each participant's path down the ladder is fully determined by the ladder rules — follow the vertical line down, and step sideways whenever you hit a rung. No participant is more likely to land on any particular outcome.

No. You have to click a name to reveal its connected result. That makes it good for a group activity where everyone clicks their own name in turn to find out.

The ladder uses whichever list is shorter. It's best to enter the same number of participants and outcomes.

No. Everything runs entirely in your browser and is never sent to or stored on a server.

There's no hard limit, but a wider ladder may require horizontal scrolling on smaller screens. It's easiest to read with somewhere between 2 and 10 participants.
